The following notes have been provided to ensure your referral runs as smoothly as possible.referral_info_sheet_-dog-1_2009

A referral appointment has been made for your pet because we felt that it would be helpful to have another opinion and assistance regarding this condition from someone with extensive experience of the condition.

Treatment may also involve specialized expertise and/or equipment and procedures that may not be available at our practice.

Tips for your appointment:

  • Do not be late.
  • If you discover that you will be running late for your appointment, please call the referral center at the number above.
  • Be prepared to pay for the consultation or procedure at the time of your visit. We try to give you a cost estimate based on what the referral center has told us. If you would like a more precise estimate please do not hesitate to call the referral center for assistance.
  • If you have pet insurance, be sure to take all of your forms and policy information with you. Payment is still likely to be expected at the time of your visit. If you have any questions about payment options or pet insurance, please call the referral center before your appointment.

What happens after the appointment?

The specialist will explain the procedure and discuss the estimated cost and probable outcome.

After the examination and consultation, we will also receive a report and opinion from the consulting specialist. The specialist should tell you when to call and make an appointment for any follow-up care, either with their practice or at our hospital.  If you have any further questions or concerns, please feel free to call us or the referral practice.
